3/15/2023 EnchantaVaughn Approved Behavior: Upon arrival to the home setting, Client displayed good social skills throughout the session. Client was supported for his cooperation and wanting to address challenges in school and at home. Client admits to taking his frustration out on others and not giving his parents the respect, they deserve. Behavior: During the second portion of the session, Client was defiant of QMHP-Cs prompts 4-5 times. Client blamed his mom for his negative behaviors and appeared unapologetic for participating in disruptive actions. Behavior: During the last portion of the session, client was confronted about his impulsive conduct, his demeanor became rebellious, disobedient and noncompliant, as his personality became more and more defiant, as he began to talk back to the authority figure without any remorse or empathy as this information was communicated to QMHP at the beginning of the session. Intervention: QMHP-C reviewed Client’s ISP objectives with him in efforts to reinforce his commitment to achieving them on a daily basis. QMHP-C aided Client in recognizing his target behaviors in efforts to reinforce the importance of complying with social norms while in school, at home and in the community. QMHP-C facilitated activity to allow Client insight on conflict resolution skills to utilize in social settings. QMHP-C provided strategies such as positive reframing. Intervention: QMHP-C assisted Client with practicing reframing negative thoughts with more positive thoughts in efforts to reduce impulsive reactions to situations that usually trigger Client's negative expressions. QMHP-C coached Client to acknowledge and list his strengths, resilience, and the positive outcomes that often occur when he makes informed decisions opposed to impulsive ones. Intervention: QMHP-C verbally identified Client's impulsiveness to Client and encouraged Client to use self-control and decrease hyperactive behaviors in the school setting that have been reported by his teachers on a weekly basis. QMHP-C processed activity with Client prior to ending session in efforts to strengthen his prosocial behaviors in the home, at school, and in the community. Response: Client was receptive to all interventions given by QMHP-C. Client displayed appropriate understanding of exercise. Client was attentive while counselor assisted him with reviewing his schedule and preparing for the next day. Client was able to recognize his target behaviors and implement a plan to reduce behaviors that were not helping him with his treatment. Response: Client was very open and honest during his self-expression activity. Client displayed full participation in the session, as observed. Client demonstrated excellent understanding of being able to resolve personal conflicts within the home and community. Client was able to conduct positive reframing skills with guidance from QMHP-C. Client was observed communicating positive thoughts and avoiding negative thinking habits by changing “can’t” to “can” and “idk” to “let me try”. Response: Client listed strengths that he finds rewarding and beneficial to his daily functioning life. Client shared that he was excited for his future and could not wait until he is old enough to have his own room and do big boy stuff in the home so that he can give his mom a break. Client smiled at the thought of this happening soon. Client displayed self-control throughout the session and improvements with displaying pro social behaviors.
